Housing Counseling Agency Disclosure Statement

Please read the following statements carefully. To simplify, the singular is also used if the plural can be applied. I understand that CCCS offers different types of services, including: financial assessments, a debt management program, bankruptcy counseling and education, various financial education courses, and housing advice and education, including: financial management/budget counseling, home renovation and rehabilitation counseling, mortgage crime and default solutions counseling, pre-purchase counselling, rental counselling, homeless counselling, home buyer education prior to workshops on buying, financing, budgeting and credit education, fair housing education before purchase, and predatory loan training. I understand that following the CCCS Homeownership Assessment, Counselling and Education programs does not guarantee that a lender will approve a loan application that I submit to a lender. I also understand that completing the CCCS Homeownership Assessment, Counselling and Education programs and referring the CCCS to a lender is not an obligation of the lender to provide me with a loan. I understand that only the lender can make such a commitment after receiving a loan application that I have completed. I understand that the purpose of the net worth assessment, counselling and education programs offered by the Black Hills Consumer Credit Counselling Service (CCCS) is to advise me and/or provide me with information about mortgage eligibility and the process of buying and financing the home. I understand that the CCCS may give me recommendations regarding lenders or programs that can meet my individual needs. I understand that I have no obligation to receive, purchase or use any other services offered by agencies or their exclusive partners to receive housing counselling services. I understand that the CCCS receives funding from a variety of sources, including HUD grants, debt management revenues, United Way, fair share of creditors and community support. I understand that while the agencies work with many other organizations, they do not have business practices or partnerships that would constitute a conflict of interest.

The organizations each have conflict of interest guidelines. Residents of South Dakota and Wyoming: CCCS charges a one-time installation fee of between $50 and $95, which is determined using a sliding scale based on the client`s gross monthly household income and family size. After signing up for the DMP, Navicore Solutions charges a monthly fee of up to $60. I understand that if I choose a debt management program (“DMP”) with CCCS and Navicore Solutions (collectively, the “Agencies”), a fee will be charged for that service: I have read and understood my rights and obligations under the documentation I have uploaded regarding Customer`s rights and obligations. I also understand that I can verify this information by scrolling down on this website and clicking on the Privacy Policy link. I understand that if I am not satisfied with one of the agencies` services, I can use the complaint procedure. I understand that both organizations have a quality control plan and quality improvement policies and procedures in place to identify, address and mitigate conflicts of interest and meet HUD requirements. Nebraska Residents: CCCS charges a one-time installation fee of $25, which will be credited to your total fee. Upon filing with DMP, Navicore Solutions will charge the lower amount of 15% of the customer`s debt to be paid, or $60.00 amortized over the life of the plan. As an organization that receives federal funding from HUD, we are required to collect the following demographic information about participants in our programs. Credit When Credit is Due is a copyrighted program.

Copying any part of the book or PowerPoints is strictly prohibited and is a violation of the law. Any treatment of graduates as credit graduates when credit is due without sending them through our organization is a violation of this program. Completion of the Credit program when credit is due can only be verified with our organization. At some point in the future, my information may be used for confidential research, quality assurance/training and/or HUD purposes, or a neutral third party may contact me to request and evaluate agency services. I understand that the organizations each have guidelines for business ethics and professional ethics. All employees are aware of and adhere to the Code of Ethics, which is posted in every office. I understand that CCCS/BH receives federal funding through Housing and Urban Development (HUD) and is required to collect demographic information about program participants and to share some of my personal information with HUD program administrators or their representatives for the purposes of monitoring, compliance and evaluation of the program. It is my understanding that both organizations are registered as 501(c)(3) nonprofits and are each independently managed by separate voluntary boards of directors. The services of organizations do not serve the private interest of their boards of directors, employees or consultants.

The agencies have policies in place that prohibit the practice of referring applicants or clients to a private practice where staff, consultants or immediate family members of the agency`s staff and consultants may operate. . . .
